Creating a survey/questionnaire

1. Navigate to your course

2. Click the "Edit mode" button.

3. Click "Add an activity or resource"

4. Click "Questionnaire"

5. Click the "Name" field.

6. Enter the name of your survey/questionnaire

7. Enter a name and description in the text box

8. Click "Activity completion"

9. Click this dropdown.

10. Click the "Student must submit this questionnaire to complete it" field.

11. Click this button.

12. Click "Add questions"

13. Click this dropdown.

14. Click this button.

15. Click the "Question Name" field.

16. Add your question here

17. Enter your question in this text box

18. Click the "Possible answers" field.

19. Type the list of answers that the student will be scoring their knowledge on

20. Click this button.

21. Click "Preview"

22. Click this button.

23. Repeat the process above for any new questions that you wish to add to your survey
